These are various exit ticket prompts you can skip around in and ask the students in different ways. To make this formal you could ask the students to write a sentence or paragraph depending on age which would make it quanitative. Or you could ask questions to the students and have them verbally answer which would make it informal. Depending on the difficulty of the question this could work for different age groups but most of the questions would fall under the middle school range.
